Case ManagementOur CAP services allow us to meet the needs of our patients who are actually on a nursing home level of care. We provide medical social work, in-home aides and supplies. These services also allow our patients to remain at home to receive the same types of care that they would have, had they been placed in a nursing home facility or institution. Our social workers work closely with the Department of Social Services, as Medicaid reimburses this program. There is a waiting list for patients to be admitted into our CAP Program.
