Overview

When a hospice patient is admitted, each patient is assigned a team of professionals to ensure that their remainder of days are as best as they can be. Our hospice team is derived of the following:

- A Hospice Nurse
- A Physician
- A Social Worker
- A Chaplain
- A Volunteer
- An In- Home Aide
